MAKE A REFERRAL
Permanent Supportive Housing
HOW’s intake process requires case managers from referring agencies to initiate the application process. Please note that because we do not accept self referrals, phone calls should be from the referring agency staff rather than the applicant herself. The referring agent should also submit proof of the applicants’ homeless status (for example a letter from an emergency shelter stating the applicant is currently staying there), and either a recent paycheck stub OR letter of disability from the applicants’ Medical Doctor or Psychiatrist.

The first stage for evaluation is the application*. Upon review of the application, if an applicant meets the minimum criteria and if there is space in the program for placement or waitlist, the referral agency will be contacted for an interview with the applicant. Acceptance or denial is based on the interviews which are conducted by a Case Manager and a Recovery Specialist (if applicable). Determinations are generally made within 10 business days of the interview.

Homelessness Prevention and Rapid Re-Housing (HPRP)
HOW’s Homelessness Prevention Program is a partnership with both the Emergency Fund and the City of Chicago, and HOW runs its program with Lakeside CDC. As a result, this program has a different intake process than HOW’s Permanent Supportive Housing Program. It is important to note that clients will be assigned to a HPRP participating agency, which may or may not be HOW.
There are two ways to access HPRP:
1. Call 311 and ask for the Homlessness Prevention Department
